Our organization has earned the tier two recognition as part of the AHCA/NCAL Quality Initiative Recognition Program for achieving the following Quality Initiative goals:
Our staff at Cloverdale Rehabilitation and Nursing Center strive to meet these high quality measures on a daily basis. While many admissions/readmissions into the hospital are necessary, some can be avoided. Preventable hospital admissions/readmissions place a significant physical and emotional burden on patients and their families. Avoiding these transfers will alleviate this burden and will allow residents the comfort and well being of sleeping in their own beds.
We will continue to strive to exceed these Quality Initiative goals for the improvement of the lives of the residents we serve, which is what the Quality Initiative is ultimately about.
